Primary care membership program. Our goal is for you to remain healthy and active throughout your life. We like to think of this as healthspan as opposed to simply living longer. You deserve a trusted physician for guidance-a physician who is knowledgeable, accessible, and willing to help coordinate your health care. The annual membership fee includes your annual comprehensive examination; all routine office visits; 24/7 access via telephone, email, or text; medication management including drug interaction surveillance; coordination of medical or surgical specialist visits, and other administrative needs. Patients are welcome to have their ongoing primary care with Dr. Abrams, Dr. Mohtadi, or Dr. Diaz. Primary care is included in the program fee.
Annual comprehensive examination.
- An unhurried, all-inclusive medical history and physical examination. Our exam focuses on lifestyle, nutrition, fitness, family health history, and medications (including vitamins and supplements). We use computer software to detect medication interactions.
- In-depth consultation. You will have all the time you need with your physician to review results, see your radiology images, and most importantly, develop a personal plan for healthy living
- Portable personal health record. Your medical history and exam, lab tests, radiology reports and all other important health records will be provided for you in a bound book, as well as electronic records.
- Computerized medication management. All medications should be reviewed at least yearly to look for potential side effects or interactions. We use contemporary medication management software to accomplish this important task.
- Expansive laboratory tests. In partnership with the Quest Diagnostics, Cleveland Heart Lab and GRAIL we perform advanced blood screening for heart and vascular risks, diabetes, liver and kidney dysfunction, metabolic problems and cancer.
- Multi-cancer screening test. The Galleri test (from GRAIL, Inc.) is a multi-cancer early detection test that can reveal more than 50 types of cancer, many of which are not commonly screened for today.
- Coronary artery calcium detection. Using state-of-the-art, ultra-fast CT imaging technology we examine your coronary arteries quickly and safely for early signs of atherosclerosis.
- Carotid artery ultrasound. With highly sensitive ultrasound equipment your carotid arteries can be seen and measured. The carotid arteries are vital because they supply your brain with oxygen-rich blood.
- Abdominal and pelvic ultrasound or MRI (your choice). Using this form of imaging we will be able to evaluate your abdominal aorta for aneurysms as well as your pancreas, liver, gallbladder, bile ducts, kidneys, spleen, bladder, and intra-abdominal fat. These types of exams do not expose you to any radiation.
- Electrocardiogram (EKG). A digital EKG records your heart rhythm and electrical patterns. Your EKG will be available on your personal health record if it’s ever needed in the future.
- Audiometry. A digital hearing test can identify early, correctable hearing loss.
